Apply for a card online

Use this Registration Form to apply for an adult library card online. You may come to the library to pick up your card, just be sure to bring two forms of ID with your current street address. Please allow two days for your library card to be processed.

    ($1.00 fee for replacement cards)
  • Use for street address and apartment number
  • Use for P.O. Box used for mailing
  • Ex. 845-563-3600 or 845-563-3600 x 1234. If you do not have a telephone, please enter "No Telephone/Unlisted Telephone".
  • This field is required for online registration. Your email address will be used for email notification of holds & overdues.
